Professional Referrals 

Pain Pals GY support adults 16+ who suffer with chronic pain, disabilities and mental health problems. 

We are NOT able to support those who need assistance with children or child based benefits. 

If you work for a professional or charitable organisation and wish to refer someone to Pain Pals GY.  

Please make sure you can provide :  

• Necessary contact details for the person being referred: Pronouns, Name, Address, Phone number, Email

• The services users or carers preferred means of contact, including days/times when they will be available.

• If the service user has any care plans, safeguarding needs or risk assessments. These should be include.

• Should your organisation need to remain in contact with Pain Pals GY for the purpose of monitoring the service user. Pain Pals GY  will need  written confirmation of the required recording and reporting procedures.
